Now to decide if I want to be put under or not, it's not the tooth pulling I am worried about but the screw they want to drill into my jaw for a permanant fake one they will be putting in.

Tell that to my wife. I haven't had a cavity yet, but if any of my back teeth ever have to get pulled, they wont be replaced until I am old and need dentures.
