When you're nailing a wall together, it's laying down flat. The studs are layed on the top plate, which is stood on edge. Then you work down the wall, grabbing a stud with one hand, turning it edge up, setting it on layout, and then shooting through the top plate into the top of the stud. Sometimes, when you're going fast, your hand will "creep up" the studs, so that the hand holding it is only an inch or so below the top plate. And then if you bump fire the gun on the edge of the top plate, the wood will blow out, and the nail goes into your hand. Usually try to keep your hand about a foot or so down the stud, to keep this from happening, but it doesn't always work that way when you get rolling.
